Instructions for requests by: Fax | Mail
We currently have three options for Alaska residents and former Alaska residents to obtain a copy of their driving record. You may request the record by mail, fax or by going to any DMV and showing proper identification. Driver license records are confidential; therefore, you may only obtain your personal record, unless you have a signed release to obtain a record for another person. Parents may obtain records for their minor children. Driver's license record checks cannot be conducted by telephone due to the requirements of producing identification and the payment of the research fee.
Be sure to specify in your request whether you need the record for employment purposes, insurance, "clearance letter" or other. This will help us to send you the right document for your specific purpose.
Please use the following form to apply for a record by fax:
Please do not fax your request more than once. If you faxed your request and have not received it in a reasonable time period (Please allow 2 business days), send a message containing the following information: Legal name, Date of Request and the Fax number that the request was sent to. We can check to assure it was received and provide further information and instructions.
You may obtain a driving record at any DMV office or mail your request to the address shown below. The following items are required to obtain your record:
